چکیده

A giant aneurysm of the distal arch after stenting and carotid-subclavian bypass by type B dissection. A 64-year old female with type B dissection and expanding distal arch who underwent stent implantation covering the left subclavian artery and carotid-subclavian bypass 8 years ago, presented with dysphagia and distal aortic arch aneurysm. Although the aneurysm (Figs 1 and 2) was excised successfully through a clamshell approach, she suffered a fatal respiratory failure. Figure 1: (A–D) Preoperative computer tomography showing the giant distal aortic arch aneurysm and the stent in the true lumen. The left subclavian artery, which was sacrificed at the initial procedure and the false lumen were open (A and B), resulting in a 7-cm aneurysm (C and D).

Aortic arch/elephant trunk procedure with Sienna(TM) graft and endovascular stenting of thoraco-abdominal aorta for treatment of complex chronic dissection.

Aneurismal dilatation of the remaining thoracic aorta after ascending aortic interposition grafting for type 'A' aortic dissection is not uncommon. For such complex cases, one treatment option is total arch replacement and elephant trunk procedure with the Sienna(TM) collared graft (Vascutek, Inchinnan, UK) technique followed by a staged thoracic endovascular aortic repair (TEVAR). The video il...

Successful rescue of the ruptured chronic B dissecting aneurysm after endovascular stent-graft with supraclavicular carotid artery graft cannulation

Method A 44-year-old patient underwent the TEVAR with chimney technique of left subclavian artery in a life-threatening situation of type B chronic dissecting aneurysm with rupture. A follow-up CT scan showed the proximal type I endoleak with high pressurized aneurysm 5 days later. The graft explanation and open repair were performed emergently. Via a left supraclavicular incision, the end-to s...

Reoperation for a giant arch anastomotic pseudoaneurysm eleven years after total arch replacement with island reconstruction

BACKGROUND The long-term effects of some surgical treatment procedures of arch replacement for aortic dissection or aortic aneurysm are unknown. CASE PRESENTATION The present study reports the case of a 68-year-old man admitted to our hospital for aortic arch anastomotic pseudoaneurysm with concomitant aortic root enlargement and coronary artery stenosis. Eleven years ago, at the age of 56 ye...

Successful Treatment of a Ruptured Aortic Arch Aneurysm Using a Hybrid Procedure

Aortic rupture has a high mortality rate and can be considered a medical emergency. The standard treatment for aortic rupture is surgical repair. An aortic stent graft for a ruptured descending aorta is considered an effective alternative treatment. However, an aortic stent graft is difficult when the aortic aneurysm is in the aortic arch due to supra-aortic vessels. We report on a patient with...

Surgical Management of Aorto-Esophageal Fistula as a Late Complication after Graft Replacement for Acute Aortic Dissection.

A 49-year-old male presented with chills and a fever. Five years previously, he underwent ascending aorta and aortic arch replacement using the elephant trunk technique for DeBakey type 1 aortic dissection. The preoperative evaluation found an esophago-paraprosthetic fistula between the prosthetic graft and the esophagus. Multiple-stage surgery was performed with appropriate antibiotic and anti...
